Consent and Agreement to Background Check

I, the undersigned applicant, understand that as part of the volunteer application process with BASE Arizona and the Afro Scouts Youth Program, I am required to consent to a background check. This background check will include, but may not be limited to, verification of personal information, criminal history, and sex offender registry status.

By signing this agreement, I authorize BASE Arizona and its authorized representatives to obtain any necessary information from law enforcement agencies, state or federal agencies, or other relevant sources to complete this process.

I understand that the results of the background check will be used solely for the purpose of determining my eligibility to volunteer with the BASE Arizona Afro Scouts Youth Program. I further acknowledge that all information obtained will be handled with the utmost confidentiality in accordance with applicable laws and BASE Arizona policies.

I certify that all information I have provided in this application is true and complete to the best of my knowledge. I also understand that providing false or misleading information may result in the rejection of my application or immediate termination of my volunteer role if already approved.
